The organisation is looking to recruit a full time Service Coordinator to work within and manage supported living services for people with learning disabilities.

The successful applicant will have:

• A commitment to using person centered approaches in your work to achieve great outcomes for the people we support.
• Previous relevant experience, knowledge and a good understanding of working with people with a range of support needs.
• Skills in assessment and support planning, person-centered planning and the use of positive behaviour support strategies.
• Excellent communication skills that enable you to liaise effectively with professionals, colleagues, support staff and to build warm, trusting relationships with people supported and their families.
• Rota management experience
• Knowledge of relevant legislations and CQC requirements
• NVQ level 3 in Health and Social Care or be willing to work towards
